Athletic Forms
All Athletes must have the following on file with the Athletic Department before they can participate in any Sport. NCISAA Pre Participation and Exam Form, NCISAA Athlete Covid Questionnaire, NCISAA Consent to participate form, HCA Handbook, HCA Concussion Awareness Form, and the Tate Center Authorization Form. Forms must be renewed each year. The HCA Waiver Form
